What Is Polycystic Ovary Syndrome (PCOS)?
Polycystic Ovary Syndrome (PCOS), also known as polycystic ovarian syndrome, is an endocrine disorder characterized by reproductive dysfunction and abnormal glucose metabolism, driven by an interplay of genetic and environmental factors. PCOS typically manifests during adolescence, peaks during reproductive years, and can persist into perimenopause.
The core features of PCOS include ovulatory dysfunction, hyperandrogenism (elevated androgen levels), and polycystic ovarian morphology on ultrasound. Clinical manifestations vary widely among individuals but commonly include:
- Oligomenorrhea (infrequent menstruation) or amenorrhea
- Infertility or subfertility
- Hirsutism (excess hair growth in androgen-dependent areas)
- Androgenic alopecia (scalp hair thinning)
- Recurrent acne
- Obesity or unexplained weight gain
- Impaired glucose metabolism
- Enlarged ovaries with multiple small follicles
It is important to note that PCOS presents heterogeneously — patients may exhibit only one or two of these symptoms. Many also present with concurrent metabolic abnormalities such as obesity, insulin resistance, hyperinsulinemia, and dyslipidemia.
PCOS Epidemiology in China
A large-scale epidemiological study conducted between 2007 and 2011 involving 15,924 Han Chinese women aged 19–45 revealed that the prevalence of PCOS among Chinese women was 5.61%, with hyperandrogenism affecting up to 85% of diagnosed PCOS patients [1]. Alarmingly, only 9.61% of affected women were aware they might have an endocrine or gynecological disorder, and merely 5.9% of PCOS patients received treatment. Even among those who underwent standardized therapy, treatment outcomes were often suboptimal — highlighting a critical gap in PCOS awareness, early diagnosis, and effective management.
What Causes Polycystic Ovary Syndrome?
PCOS arises from a complex interaction between genetic predisposition and epigenetic modifications. While the exact mechanisms remain incompletely understood, current research identifies several key contributing factors to PCOS etiology:
1. Genetic Factors in PCOS
Genetic predisposition is considered a primary driver of PCOS. Abnormalities in multiple genes within the hypothalamic-pituitary-gonadal (HPG) axis directly and indirectly contribute to PCOS pathogenesis. A 2018 meta-analysis of European genome-wide association studies (GWAS) by Day F et al. identified 14 genetic loci associated with PCOS [2]. The analysis further revealed shared genetic architecture between PCOS and conditions including obesity, type 2 diabetes (T2D), menopause timing, and depression.
2. Hyperandrogenism
Hyperandrogenism is one of the diagnostic criteria for PCOS. Insulin resistance, obesity, and elevated androgen levels are inextricably linked in PCOS pathophysiology. Androgens play a dual role in folliculogenesis: low androgen levels promote follicular growth, while high androgen levels stimulate granulosa cells to secrete excessive anti-Müllerian hormone (AMH), which in turn suppresses follicular development and contributes to anovulation.
3. Insulin Resistance
Studies indicate that the prevalence of insulin resistance in PCOS ranges from 20% to 40%, and up to 95% of obese PCOS patients exhibit insulin resistance. PCOS insulin resistance plays a critical pathophysiological role in both reproductive and metabolic disturbances and is closely linked to long-term metabolic complications of PCOS, including type 2 diabetes and cardiovascular disease.
4. Chronic Low-Grade Inflammation
Chronic inflammation is increasingly recognized as a pathological process in PCOS. Emerging evidence suggests that chronic low-grade inflammation contributes significantly to the development and progression of long-term PCOS complications, including metabolic syndrome and elevated cardiovascular risk.
5. Gut Microbiota Dysbiosis
Gut microbiota dysbiosis can activate the host immune system, triggering systemic chronic inflammation. This inflammatory state impairs insulin receptor function, leading to insulin resistance. The resulting hyperinsulinemia may interfere with follicular development and stimulate ovarian theca cells to produce excessive androgens, thereby participating in PCOS pathogenesis through the gut-ovary axis.
6. Endocrine Abnormalities
Patients with PCOS are more prone to autoimmune thyroid disease (AITD). PCOS complicated by insulin resistance or infertility may further disrupt thyroid function. Conversely, AITD may increase the risk of endocrine metabolic disorders, infertility, and adverse pregnancy outcomes in PCOS patients, underscoring the importance of thyroid screening in PCOS management.
7. Metabolic Disorders
Approximately 60%–70% of women with PCOS are overweight or obese. Obese PCOS patients exhibit more severe endocrine hormone disturbances and metabolic dysfunction, including more pronounced insulin resistance, higher hyperinsulinemia levels, and enhanced chronic inflammatory responses. These metabolic abnormalities can further affect hypothalamic-pituitary-ovarian (HPO) axis function and alter gut microbiota composition, thereby perpetuating the PCOS metabolic cycle.
8. Psychological Factors
Work-related stress, chronic mental tension, and anxiety can induce neuroendocrine disturbances, serving as potential triggers for PCOS onset and exacerbation. This highlights the bidirectional relationship between PCOS and mental health.
How Does PCOS Affect Women and Their Offspring?
PCOS is not merely a gynecological disorder — it is a systemic condition with health implications that span a woman's entire lifetime. Understanding the long-term health impacts of PCOS is essential for comprehensive patient management.
Effects on Women's Health
Infertility: PCOS is one of the most common causes of anovulatory infertility. Follicular development is arrested at the preantral stage, resulting in fluid-filled cysts and the absence of ovulation. PCOS infertility accounts for a significant proportion of infertility cases worldwide.
Increased Risk of Pregnancy Complications: Women with PCOS who conceive face a higher risk of pregnancy-related complications, including gestational hypertension, gestational diabetes mellitus (GDM), and polyhydramnios compared to women without PCOS.
Elevated Risk of Miscarriage: PCOS and miscarriage are linked through luteal phase insufficiency and reduced endometrial receptivity, both of which contribute to higher miscarriage rates.
Poorer Pregnancy Outcomes in Obese PCOS: Obese PCOS patients tend to have worse pregnancy outcomes compared to their non-obese counterparts, including higher rates of preterm delivery and neonatal complications.
Endometrial Pathology: Without ovulation, there is no corpus luteum formation and therefore no progesterone production. The unopposed estrogen stimulation of the endometrium over time increases the risk of endometrial hyperplasia and potentially endometrial carcinoma.
Comorbidities: PCOS is associated with hyperandrogenism-related symptoms (acne, hirsutism), metabolic abnormalities (type 2 diabetes, dyslipidemia), and cardiovascular risk factors.
Psychological Burden: Irregular menstruation, infertility, severe acne, and obesity can significantly impact mental health, emotional well-being, and quality of life in women with PCOS.
Effects on Offspring Health
- Female offspring of women with PCOS have an increased risk of developing PCOS themselves in adulthood, suggesting a transgenerational component to PCOS inheritance.
- Offspring are more susceptible to insulin and glucose metabolism disorders, including impaired glucose tolerance and early-onset metabolic syndrome, compared to the general population.
